Surgical Pathology Criteria

For Patients

Getting a second pathologist's opinion can fill an important emotional need as well as establishing medical needs and treatment goals. When a second opinion confirms initial findings, it can provide reassurance and feelings of acceptance for the patient, and may reduce anxiety and uncertainty.

If you are not a Stanford patient and you wish a second opinion from Stanford Pathology but are not planning to receive care at Stanford, we require that your physician or pathologist request your second opinion by completing our consultation request form.

It is important to ensure that the request form is complete in order to avoid delays in processing.

Attention to referring physician’s name, billing information and insurance authorizations is extremely important. Please include a copy of the consultation request form with all case materials. If the consultation request form is incomplete, the slides will not be reviewed until the request form is complete. Send the completed form, materials to be reviewed, and a copy of the surgical pathology report to the address below. Overnight delivery is preferred.
