Residency Program Personnel
Ambulatory Care
Alice Wen, Pharm.D. received her degree at University of Southern California in 1998 and completed an ambulatory care residency at Kaiser Orange County. She worked as internal medicine pharmacist at VA Palo Alto before coming to Stanford in 2002. She currently works as an ambulatory care pharmacist specializing in anticoagulation. She has also achieved National Certification as an Anticoagulation Care Provider (CACP).
Vanessa Guzman, Pharm.D., BCACP, CDE, is an Advanced Practice Pharmacist practicing in the Primary Care setting. She earned her BS in Chemistry from the Stanford University and her PharmD from the University of California, San Francisco School of Pharmacy. She completed her pharmacy practice residency at Stanford Health Care where she rotated through a variety of practice settings including solid organ transplant, emergency medicine, critical care, and ambulatory care. During her time at Stanford Health Care, she has led an initiative to conceptualize and implemented innovative clinical pharmacy services at Stanford’s Outpatient Clinics.
Victoria Lee, Pharm.D., BCPS, received her Doctor of pharmacy degree from the University of the Pacific. She completed a pharmacy practice residency with ambulatory care emphasis at Santa Clara Valley Medical Center (SCVMC). After the completion of her training, she practiced as an ambulatory care clinical pharmacist at SCVMC and Highland Hospital prior to joining Stanford in 2016. She currently works as an ambulatory care pharmacist at the Multispecialty Clinics in Emeryville.
Nancy Shin, Pharm.D., CDE received her undergraduate degree at the University of California, Berkeley and her pharmacy degree from the University of California, San Francisco. She completed pharmacy practice residency at the VA Palo Alto Healthcare system, and was an ambulatory care clinical pharmacist at the VA before joining Stanford in 2016. She currently works at Stanford's Primary Care clinic in Santa Clara. She is a Certified Diabetes Educator.
Agnes Chou, Pharm.D.,BCPS, BCACP, CACP received her doctorate in pharmacy degree from Long Island University—Arnold and Marie Schwartz College of Pharmacy. She went on to complete a residency with specialization in ambulatory care at The Brooklyn Hospital Center. She currently works as an ambulatory care pharmacist predominantly in the oral anticoagulation clinic (OAC).
Carmen Lu, Pharm.D., received her undergraduate degree in Integrative Biology from the University of California, Berkeley and her Doctor of Pharmacy degree from the University of the Pacific. She completed a general pharmacy practice residency at Stanford Health Care. She currently works as an ambulatory care pharmacist at the Stanford Coordinated Care clinic and the Multispecialty Clinics in Emeryville.
Bone Marrow Transplant
Ya-Hwei Cheng, MS, Pharm.D. received her MS in health system management from Union University, NY and pharmacy degree from University of Florida. She joined Stanford Hospital & Clinics in 1997. She has practiced in medicine, critical care, cardiology, orthopedics, pediatrics, oncology and investigational drug services. She is currently a clinical pharmacist specializing in BMT.
Fu-ying Lee-Lam, R.Ph. received her B.S from St. John's University, New York in 1988. She worked at Memorial Sloan Kettering for five years before moving to California. Currently she works at Stanford Health Care as a Bone Marrow Transplant pharmacist. BMT has been her focus of practice for the last 15 years.
Joanna Wang, Pharm.D. received her undergraduate degree in Biology from the University of California, Irvine and her Doctor of Pharmacy degree from the University of Southern California in 1991. She completed her residency at University of California, Los Angeles in 1992 and currently works as a clinical pharmacist in Bone Marrow Transplant & Oncology.
Ted Kim, Pharm.D., received his Pharmacy degree from the University of Texas in Austin in 2014. He completed his training in hematology/oncology and hematopoietic cellular therapy from the Methodist Hospital in San Antonio, TX. After the completion of his training, he worked under a clinical collaborative practice agreement with the Texas Transplant Institute. In addition to his clinical practice, he worked with other HCA institutions to create multi-institutional standard operating procedures, transplant conditioning algorithms, as well as managing the oncology pharmacy team at Methodist. He joined the Stanford Healthcare BMT pharmacy team in February of 2017.
Cardiology
Kelly Matsuda, Pharm.D., BCPS (AQ Cardiology) received his pharmacy degree from the University of the Pacific. He is a clinical pharmacist specializing in cardiovascular medicine/pharmacology in the Coronary Care Unit.
Manya Sarram, Pharm.D. (Lead Pharmacist) graduated in 1988 with a B.S. in pharmacy from the University of Munich, Germany. After moving to the United States, she went on to complete a 2-year Pharm.D/residency program at the University of Washington, Seattle. She has been a clinical pharmacist in Cardiology Medicine here at Stanford since 1995. Currently she is one of the Lead Pharmacists and is dividing her time between clinical and administrative duties.
Central Operations
Erika Boetius, Pharm.D. attended UC Davis for her undergraduate years and then received her Doctor of Pharmacy from the University of Pacific in 1987. After working 11 years in retail, she was hired as the Pharmacist in Charge for the Stanford Outpatient Pharmacy in 1998. She worked 3 years in the Outpatient setting and then transferred into the Inpatient Pharmacy. She has worked as part of the management team and is currently staffing the Central IV Clean Room.
Critical Care
April Fong, Pharm.D. received her undergraduate degree from UCLA and her Doctor of Pharmacy degree from the University of Washington in 2003. She completed her pharmacy practice residency and critical care residency at Stanford Health Care. She currently works as a critical care pharmacist.
Janice Hsieh, Pharm.D. graduated from University of Southern California. She completed her general pharmacy practice residency at Kaiser Vallejo. She worked at Kaiser Redwood City for 2 years and joined Stanford's Department of Pharmacy as a critical care pharmacist in 2004.
Jamie Kuo, Pharm.D., BCCCP received her undergraduate degree from University of California, Berkeley and earned her pharmacist degree from the University of California, San Francisco. She completed her PGY-1 pharmacy practice residency and PGY-2 critical care specialty residency at Stanford Health Care. She currently works as a clinical pharmacist in critical care medicine.
Cherwyn Nguyen , Pharm.D., BCCCP graduated from University of California, San Francisco and completed her pharmacy practice residency and a critical care residency at Stanford Health Care. She is currently a clinical pharmacist specializing in critical care medicine.
Vitaliy Nikitenko, Pharm.D., BCCCP received his undergraduate degree from University of California, San Diego and earned his doctorate in pharmacy from the University of Maryland. He completed his PGY-1 pharmacy practice residency and PGY-2 critical care specialty residency at Loma Linda University Medical Center. He currently works as a clinical pharmacist in critical care medicine.
Mike Schoshinski, Pharm.D. received his undergraduate degree from Marquette University and his pharmacy degree from the University of California, San Francisco in 2001. He completed his pharmacy practice residency at Stanford Health Care and currently works as a critical care pharmacist.
Craig Sterling, Pharm.D. received his undergraduate degree from Humboldt State University in Chemistry and his pharmacy degree from the University of Southern California. He completed his pharmacy practice residency at Ohio State University and currently works as a critical care pharmacist.
Elizna van Zyl, Pharm.D., BCCCP received her Pharmacy degree from the University of the Pacific in 2014 and then completed both her general and critical care specialty residencies at Loma Linda University Medical Center in Southern California’s Inland Empire. Prior to joining the Stanford Healthcare pharmacy team, she had worked at Kaiser Permanente as an intern as well as a per diem pharmacist at Loma Linda during her residencies. She works in the ICU and OR pharmacies as a clinical pharmacist.
Emergency Medicine
Christine Reynolds, Pharm.D., BCPS, graduated from Ohio Northern University in 2012. She completed her PGY-1 pharmacy practice residency at Methodist Hospital in San Antonio, Texas where she created their emergency department pharmacist position. She worked as an emergency medicine clinical pharmacist at Methodist Hospital for two years before joining the emergency department specialty service at Stanford Health Care.
Christian Hamm, Pharm.D., BCPS received his B.A in Biology from the University of Hawaii. He then moved to Seattle to study pharmacy at the University of Washington. His PGY-1 was completed at MultiCare Health Systems in Tacoma, Washington - where his resident project of implementing an Emergency Department pharmacist was able to show cost-savings and medication safety to the hospital. This led to the approval of several new pharmacist positions. After eight years leading the ER service there, he transitioned to be part of the Stanford ER pharmacy team.
Brian Dang, Pharm.D., received his Doctorate of Pharmacy from the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences in 2014. He completed his PGY-1 Pharmacy Practice Residency at University Medical Center of Southern Nevada in Las Vegas, Nevada and went on to complete his PGY-2 Emergency Medicine Pharmacy Residency at the University of California, Davis Medical Center in Sacramento, California. He is currently working as an emergency medicine pharmacist at Stanford Health Care.
Daniel Totleben, Pharm.D., BCPS received his Doctor of Pharmacy from Duquesne University in 2010. He practiced at MedStar Washington Hospital Center and Naval Medical Center San Diego before coming to Stanford in 2016. He is currently working as an emergency medicine clinical pharmacist at Stanford Health Care.
Ivan Mok, Pharm.D., BCPS received his pharmacy education at UCSF 2013 and completed his PGY1 at San Francisco General Hospital. He practiced at SFGH in the emergency room and inpatient pharmacy prior to joining the emergency medicine pharmacy group at Stanford Health Care.
Hematology/Oncology
Alice Chang, Pharm.D. received her degree from the University of Colorado in 1998. She completed an ambulatory care residency at University of Illinois at Chicago and worked at Olive-View UCLA Medical Center for 4 years before joining Stanford Health Care in 2003. She is currently practicing as a clinical oncology pharmacist in the outpatient Cancer Center.
Magdalena Seyer, Pharm.D., BCPS received her degree from the University of California, San Francisco in 2006. She completed a pharmacy practice residency at Stanford Health Care. She worked as a clinical pharmacist for 2 years before transitioning into hematology and oncology at Stanford health Care.
Latanya Dean, Pharm.D., BCOP (Assistant Director Hematology/Oncology) received her degree from the University of Washington in 2001. She completed a general practice residency at Stanford Health Care in June 2002. After residency she accepted a clinical position at SHC and worked in the Cancer Center as a Clinical Pharmacist, then as a Lead Pharmacist overseeing the inpatient and outpatient oncology pharmacy services.
Rawmina Keshavarzi, Pharm.D., BCOP received her degree from the University of Maryland in 2013. She completed her pharmacy practice residency and PGY-2 oncology pharmacy residency at UC Davis Medical Center. After completing her residency, she came to Stanford Health Care where she now practices as a clinical oncology pharmacist in the outpatient South Bay Cancer Center.
Eun-Jeong Kim, Pharm.D., MS, BCOP, BCPS received her degree from the University of Florida in 2003. She completed her pharmacy practice residency at Shands Jacksonville/The University of Florida Academic Health Center and PGY-2 oncology pharmacy residency at University of Washington Medical Center. She worked as a clinical oncology pharmacist at Hematology/Oncology Ambulatory Clinic at Kaiser Permanente and Sutter Health for seven years before joining the hematology/oncology specialty area at Stanford Health Care.
Ann Le, Pharm.D., BCPS received her degree from the University of California, San Francisco in 2009. She completed a pharmacy practice residency at University of California, San Diego. She completed a hematology/oncology residency at Stanford Health Care in 2010 and currently works as a clinical pharmacist specializing in hematology and oncology.
Samit Patel, Pharm.D., BCOP graduated from the University of Houston in 2007. He completed his pharmacy practice residency at Presbyterian Hospital of Dallas/Texas Tech University Health Science Center. He completed a hematology/oncology residency at Stanford Health Care and currently works as a clinical pharmacist in hematology/oncology.
Faye Tsung, Pharm.D. (Lead Pharmacist) received her Bachelors in Pharmacy at the University of Toronto in 1989. In 1990, she completed an Industrial Pharmacy Residency at Eli Lilly Canada Inc. In 1994 she received her Doctor of Pharmacy degree at Philadelphia College of Pharmacy and Science. The following year she completed an oncology residency at the University of Illinois at Chicago. She joined Stanford Health Care in 2002 and currently works as a Lead Pharmacist overseeing the inpatient and outpatient oncology pharmacy services.
Abraham Chang, Pharm.D., BCOP graduated from the University of the Pacific in 2012. He completed his PGY-1 pharmacy practice residency in 2014 and PGY-2 hematology/oncology residency at Stanford in 2015. He currently works as a clinical pharmacist in hematology/oncology.
Ellen Wong, Pharm.D., BCOP, received her degree from the University of California, San Francisco in 2009. She completed a pharmacy practice residency at University of California, Davis. She worked a hematology/oncology clinical pharmacist at UC Davis Medical Center for 3 years before joining the hematology/oncology specialty area at Stanford Healthcare.
Linh Tran, Pharm.D., BCOP earned her degree from Western University of Health Sciences in 2012. She completed a general pharmacy practice residency at Baptist Memorial Hospital in Memphis, TN in 2013 and a hematology/oncology residency at Stanford Hospital and Clinics in 2014. Currently, she works as a clinical pharmacist specializing in hematology and oncology.
Stephanie Tse, Pharm.D., BCOP received her Pharmacy degree from the University of California, San Diego in 2013. She completed her PGY-1 pharmacy practice residency and PGY-2 hematology/oncology pharmacy residency at the University of Washington Medical Center. She worked at the University of Washington & Seattle Cancer Care Alliance as part of the BMT and hematology/oncology team before joining the hematology/oncology specialty area at Stanford Healthcare.
Infectious Diseases
Lina Meng, Pharm.D., BCCCP, BCPS, graduated from University of California, San Diego in 2009. She completed her pharmacy practice residency at the University of California, San Francisco in 2010 and a critical care residency at Stanford Health Care in 2011. She currently works as a critical care pharmacist covering medical-surgical ICU and cardiology.
Emily Mui, Pharm.D., BCPS received her pharmacy degree from Northeastern University in 2008. She completed her pharmacy practice residency and infectious disease specialty residency at Brigham and Women's Hospital in 2010.
Medicine
Tiffany Wong, Pharm.D., BCPS, received her pharmacy degree from the University of California, San Diego in 2012. She completed her pharmacy practice residency at Stanford Health Care in 2013 and currently works as a clinical pharmacist in Internal Medicine.
Denise Kwong, Pharm.D., BCPS, received her Doctor of Pharmacy from the UCSD Skaggs School of Pharmacy. She completed her residency (PGY-1) at Stanford Health Care. Her main practice area is in General Medicine and her areas of interests are in Internal Medicine, Cystic Fibrosis, and Cardiac Surgery.
Practice Management
Deepak R. Sisodiya, Pharm.D., MHA, BCPS, Director of Pharmacy received his BS in Pharmaceutical Sciences from the University of Alberta and his PharmD from the University of Michigan in 2002. After completing a pharmacy practice residency at Stanford Health Care he worked as a clinical pharmacist in general medicine with an interest in infectious diseases. In 2010 he made the move into management.
Mark Klanjac, Pharm.D., BCPS, Manager of Professional Development received his Doctor of Pharmacy degree from the University of the Pacific and completed his pharmacy practice residency at Stanford Health Care in 2006. He most recently worked as a Lead pharmacist overseeing the OR and ICU areas.
Esther Kimm, Pharm.D. received her Doctor of Pharmacy degree from the University of Southern California in 1996 and subsequently completed a primary care residency at the Veterans Affairs Outpatient Clinic in Los Angeles, California. Prior to joining Stanford in 2015, Esther’s experiences range from being an ICU/Transplant pharmacist, assistant professor of pharmacy, a pediatric/neonatal pharmacist and a quality manager at various institutions including Cedars Sinai, the University of Southern California and Kaiser Permanente. She currently oversees the pharmacy quality program at Stanford Health Care.
Janjri Desai, Pharm.D., MBA, BCPS completed a joint PharmD/MBA program at University of the Pacific in 2011. She then pursued a PGY-1 general pharmacy practice residency at the VA Palo Alto Health Care System and remained there as an Inpatient/Outpatient Clinic Pharmacist, after which she took a Medicine/Surgery Pharmacist position at the San Francisco VA Medical Center. More recently, Janjri joined SHC in December 2015 as the Manager of Clinical Effectiveness and now oversees the Antimicrobial Stewardship, Medication Reconciliation, and Transitions of Care programs. Additionally, she assists with formulary management and supports the Pharmacy & Therapeutics Committee (P&T).
Martin Caponi, BS Pharm. received his undergraduate degree from the University of Michigan, Ann Arbor in 1981. Martin has over two decades of hospital pharmacy leadership experience and joined Stanford Health Care Pharmacy department in July of 2015 as Assistant Director, Inpatient Operations.
Lana G. Witt, Pharm.D., Assistant Director of Pharmacy-Outpatient Services oversees pharmacy services in ambulatory care clinics, infusion centers and outpatient surgery practices as well as SHC expansion to offsite locations. Previously she was an inpatient clinical pharmacist in the Med-Surg Intensive Care unit, worked in the Decision Support group at fdb (First Databank) and implemented the Epic EHR at Stanford as a clinical informaticist on many teams and projects including the Willow Pharmacy team, Beacon Oncology and the Epic Ambulatory team.
Psychiatry
Julie Newton, Pharm.D. (Lead Pharmacist) graduated from University of the Pacific in 1985. After working 3 years for a small independent pharmacy, she completed a general residency at VA Long Beach. She has worked for Stanford since 1989 and has staffed in several pharmacy coverage areas including the OR, pediatric ICU, and skilled nursing. In her current position she continues to work with the in-patient neurosurgery, cardiac step-down, ENT, urology, and psychiatry patient populations as well as is the Lead Pharmacist for the B2 satellite pharmacy & ED pharmacy satellite areas. She continues to precept pharmacy residents and students along with her colleagues in the B2 satellite pharmacy in the area of psychiatry.
Transitions of Care
Noah Fang, Pharm.D., BCPS received his Bachelors of Science in Medicinal Chemistry and Doctor of Pharmacy degree from the University of the Pacific. He completed his residency at NorthBay Healthcare with an emphasis in Heart Failure Transitions of Care. He currently works as the Transitions of Care Pharmacist at Stanford Health Care.
Transplant
Reid Imai, Pharm.D. received his pharmacy degree from the University of California, San Francisco and completed his pharmacy practice residency at Stanford Health Care. He currently works as a mechanical circulatory support, heart, lung, liver, and kidney transplant pharmacist at Stanford Health Care.
Roy Lee, Pharm.D., BCPS received his undergraduate degree in Molecular, Cell & Developmental Biology from the University of California, Los Angeles and his Doctor of Pharmacy degree from the University of California, San Diego in 2009. He completed his residency at Stanford Health Care in 2010 and currently works as the Heart, Lung & Heart-Lung Transplant pharmacist.
Uerica Wang, Pharm.D., BCPS received her Pharm.D. and completed a pharmacy practice residency at the University of California, San Francisco. She currently works as a kidney, liver and intestinal transplant pharmacist at Stanford Health Care.
