Applying to the Critical Care Fellowship Program

Critical Care Medicine (CCM) Fellowship Positions are available to candidates who are Board eligible or certified in Anesthesia, Internal Medicine, Neurology, Surgery, OB/GYN and Emergency Medicine. Senior surgical residents may also apply with approval of the ABS. Applicants shall have, or be able to obtain, a California medical license prior to the start of their fellowship. All fellowship start dates are in the summer (July 1 and August 1).

The two ways to apply to the CCM Fellowship program are through the Anesthesia or Medicine track. All Medicine applicants must apply through ERAS, which coordinates the application process (not a match). Anesthesia CCM applicants must follow the instructions below regarding the SF application process and match. We require that you complete and submit all of the following before your application will be reviewed.

The ACCM fellowship programs in the United States have agreed to participate in a Match administered by the San Francisco Matching Program (SF Match). NEW this year, anesthesia CCM fellowship applicants must submit all 2017-18 application documents via the SF Match Central Application System (CAS). The CAS distributes applications to fellowship training programs online. Applicants are required fill out a CAS application form, provide a set of required documents, and select programs for distribution. A separate fee, aside from the SF Match registration fee, is required for CAS distributions.

Applicants are required to have the following documents in order to apply to programs via CAS

• Complete and submit CAS application form (online);
• Three letters of reference (One letter must be from a training Program Director or Chair and the others from objective physicians (with one preferably from an intensivist) who have direct personal knowledge of your skills and ethics;
• Distribution list and payment.

Applicants invited to interview may be asked to provide the following documents separately to our review committee:

• Curriculum Vitae
• Color photo (jpg or png – high resolution)
• ECFMG Certificate and visa information, (J1 only) if applicable

Applicants accepted into the program may be required to submit the following documents:

• a copy of medical school diploma
• a copy of Dean's letter and medical school transcript
• translation of above mentioned items (if applicable)

NOTE: If asked to provide additional documents to be sent directly to the program, emailed documents are preferred and must be scanned, we will not accept photos taken of documents.

Dual 2-year applicants invited to interview will do so under the ACCM fellowship program (please see dates and descriptions above). We will coordinate the interview day to include faculty from both respective fellowships. Steps for applicants interested in both fellowships:

• Apply through SF Match CAS to the ACCM fellowship program (as stated above)
• Alert us to your interest in both fellowships (as above, please send an email to us) once your application has been started using SF Match CAS
• Applications will be reviewed by both the ACCM and ACTA review committees on a rolling basis
• Interviews will be offered through the ACCM coordinator. Candidates invited to interview will have the following options (provided for flexibility
  • Choose interview dates in which they can participate fully in the ACCM interview day (on Thursday) and the ½ day interview day for ACTA (on Friday)
  • Interview on one of the scheduled ACCM interview days (coordinators will schedule CCM interviews in the morning and ACTA interviews in the afternoon). See above regarding interview dates for ACCM
  • If the above options do not work out with your schedule you can opt to interview with the ACTA fellowship program and we will coordinate CCM interviews on that day. This day will not include the CCM overview/orientation, rounding, or didactic component of the standard ACCM interview day.
• Candidates applying for both fellowships meet the SOCCA and SCA match exception criteria. Once a match exception offer has been accepted by both the candidate and the program, we will work with the candidate to determine which fellowship will be completed first and how to proceed with the SF Match process.
• Candidates that are not given a match exception process by the agreed upon SOCCA date (TBD) will receive an email from our program stating their options to proceed with applying/ranking single fellowship positions. The ACCM match occurs first and we encourage applicants to pursue this route.

Applicants currently finishing an Internal Medicine, Emergency Medicine, or Neurology residency may apply to our program, through ERAS, for one of the two following possibilities:

• Candidates having completed Internal Medicine training may elect either a one year CCM Fellowship if they have either completed or plan to complete training in an approved medicine subspecialty, or a two year CCM Fellowship. One or the other alternative satisfies the requirements of the ABIM for CCM certification. Candidates having completed a Neurology residency complete 1 year as a CCM fellow as part of their subsequent Neurology CCM training program.
• Candidates having completed Emergency Medicine training will need to complete a 2 year CCM fellowship to satisfy the requirements of the ACEP and their agreement with the ABIM.
• Three-year Pulmonary Medicine and Critical Care Fellowship: Pulmonary Medicine programs across the country have been restructured to comply with Board requirements for one year of Critical Care Training in order to allow the candidate to become certified in both Pulmonary Medicine and CCM. At Stanford, this program will allow for a minimum of one year of Critical Care training in conjunction with Pulmonary Medicine subspecialty training. Interested applicants are encouraged to apply through ERAS.

Innovative Anesthesia - Medicine Integrated Residency Program!

Graduating medical students who are interested in a career as an Anesthesiologist - Internist are encouraged to apply to our combined Residency program approved by the ACGME in 2012 (Note: This program has its own separate match # and will be matched independently from the Stanford Anesthesia Residency match.)

Background

• The objective for the trainees choosing this innovative track is to increase their chances of succeeding in a career in Critical Care Medicine, having obtained complete training in both Anesthesiology and Internal Medicine.
• It is expected that candidates for this program would primarily be interested in a career in Critical Care Medicine, though other careers objectives (e.g. Palliative Care Medicine, Perioperative Medicine, Preoperative Assessment Clinic) might be considered.
• Since our combined Anesthesia Residency-CCM Fellowship program was so similar (medicine internship plus up to 7 months of Medicine electives during residency) to this new program, but without the ability to be board-certified in Internal Medicine, we are phasing out this program this year, 2016.

Details of the Anesthesia/ Medicine Combined Residency program

• The combined program is 5 yrs long (including internship) to be board-eligible in both Anesthesia and Internal Medicine.
• The internship year will be a full Medicine internship.
• The rest of this combined program will follow a 6 mo pattern of Anesthesia rotations, followed by 6 mo of Medicine rotations. This approach will allow for a complete integration of the two fields.
• After completion of this program, a 1 year CCM fellowship would provide board-eligibility in Critical Care Medicine.
